Your First Visit

1. Office arrival time: This will depend on whether or not paperwork was filled out at home prior to arriving in the office:
   1. Paperwork completed at home
      1. Arrive at least 10 minutes prior to your appointment. There are a few documents that require signatures on carbon copy forms.
   2. Paperwork not completed prior to the office visit:
      1. Arrive at least 30 minutes prior to your appointment.
      2. If it is your dominant arm that is injured, please bring someone with you to help you fill out the paperwork.
2. An insurance card and a driver’s license are required for identification.
   
   
3. If you were treated by another physician for the same problem: It is very helpful to obtain the following documents prior to your visit:
   1. Documentation of previous treating physicians / surgeons.
   2. All x-rays, CAT Scans, MRI’s and/or lab tests.
   3. Documentation of previous therapists.
