    1. Accommodate and treat most private insurance patients
    2. Treat Workers’ Compensation Patients
  2. Please bring your insurance card and a photo I.D. with you on your first visit.

  3. Your insurance will be verified prior to your appointment.

  4. We bill your insurance company as a courtesy to you. Payment is due at the time of service. At your request, an estimate can be provided prior to services being performed.

  5. Methods of Payment: We accept Cash, Check, Visa and Mastercard.
