- Office arrival time: This will depend on whether or not paperwork was filled out at home prior to arriving in the office:
- Paperwork completed at home:
- Arrive at least 10 minutes prior to your appointment. There are a few documents that require signatures on carbon copy forms.
- Paperwork not completed prior to the office visit:
- Arrive at least 30 minutes prior to your appointment.
- If it is your dominant arm that is injured, please bring someone with you to help you fill out the paperwork.
- An insurance card and a driver's license are required for identification.
- If you were treated by another physician for the same problem, it is very helpful to obtain the following documents prior to your visit:
- Documentation of previous treating physicians / surgeons.
- All x-rays, CAT Scans, MRI's and/or lab tests.
- Documentation of previous therapists.
- If you have splints, please bring them with you.
